Hi,
if a unit in cover is successfully attacked, the owner of that unit makes a cover-roll which normally succeeds on a 4+ for infantry and on a 5+ for vehicles. If the cover-roll is successfull, the effect of the attack is limited to disruption. If this unit is attacked successfully for a second time, the owner again makes a cover roll. If he succeeds, the unit doesn't get any additional hit-counters in addition to the face-down disrupted counter that it already received from the previous attack. If he fails, the unit gets damaged and/or destroyed.
If a unit in cover is the target of a successfull defensive fire attack , a successfull cover-roll prevents it from receiving a face-up disrupted counter.
I hope this helps.
